  • No, at this time, I am considered an out of network provider with all insurance carriers.

    I accept credit cards, debit cards, HSA cards, Venmo, and Zelle.

    Upon request, I can provide a superbill for you to submit to your insurance carrier for reimbursement. You can submit your superbill for free on Osmind, the EHR system we use.

  • 60 minute initial evaluations are $250

    90 minute initial evaluations are $375

    30 minute follow up appointments are $125

    60 minute follow up appointments are $250

    Please note we require a form of payment to be held on file for appointments. Cards will be billed after the appointment and for missed appointments/same day cancellations.

  • Yes! We offer telehealth appointments so you can attend appointments from anywhere. Telehealth appointments are conducted using HIPPA compliant zoom meetings. Links will be sent directly to you through our EMR system prior to your appointment time.

  • Luyan sees patients via telehealth only on Mondays and sees patients in office/telehealth on Thursdays. We see patients from 8:30 am to 5 pm.

  • Children are required to be present for at least a portion of their appointment time. It is vital for me to build a relationship with all my clients, no matter the age. I want everyone I work with to feel they have a voice in their care. This is particularly true for children. My goal is to help kids feel empowered to communicate their feelings and be an active participant in their mental and physical wellbeing.

  • Life happens. If you need to cancel an appointment, I ask you do so with 24 hours notice. If a cancelation is made within the same day, you will be charged 50% the rate of your scheduled service. REE medical evaluations will be charged $60 for missed appointments.

  • Refills are typically given at the time of your follow up appointment. If you are having issues with a refill or need a medication sent to a different pharmacy, we would prefer you to send a message through the EMR portal. Phone contact and secure messages are less preferred ways to receive refill requests. Please allow 48 business hours to process requests. Requests are not typically responded to on evenings and weekends.

    If you have to cancel or miss an appointment, one bridge script can be given per 12 month period at the provider’s discretion. Lost or stolen controlled substance scripts will not be replaced without a valid police report.

  • The time period between appointments vary based on a client’s needs. They typically range between 1 to 3 months.
